Job Overview

We are seeking a highly skilled and experienced Band 6 Team Leader to join our Community Nursing Team based in Deal. As a key member of our team, you will be responsible for leading and managing a caseload of patients with complex needs, providing holistic assessments, and delivering clinical interventions to meet their individual requirements.

Main Duties

As a Band 6 Team Leader, you will:

  • Undertake comprehensive assessments of patients' needs and develop personalized care plans.
  • Lead and manage a team of healthcare professionals to ensure high-quality patient care.
  • Collaborate with other healthcare teams to ensure seamless care delivery.
  • Provide health education and promote independence in patients to manage their complex long-term conditions.
  • Work closely with patients, families, and carers to ensure their needs are met and their expectations are exceeded.

Person Specification

We are looking for a candidate with:

  • Registered healthcare professional (NMC registration) with a health-related degree and evidence of relevant postgraduate development at Level 6.
  • Minimum of 5 years' experience in the area of practice, with practical experience in managing health-related projects and complex caseloads.
  • Strong influencing and negotiating skills, with the ability to work effectively with a variety of people at all organisational levels.
  • Valid driving licence in the UK and access to a vehicle.
