Position Overview

The field of Older People's Medicine (OPM) has advanced significantly to cater to the increasing needs of individuals aged 80 and above.

Our department is the largest within the healthcare organization, providing care across five specialized inpatient wards and various outpatient services. Each ward focuses on a distinct aspect of care, including Brundall, Dunston, Earsham, Loddon, and Elsing Ward.

Dunston and Brundall serve as acute care wards, addressing the complexities of cognitive impairments and intricate care requirements. Earsham is dedicated to ortho/medical care, specifically for patients undergoing surgery for hip fractures. Loddon focuses on short-term patients, typically discharging them within 3-5 days, while Elsing is a specialized unit for Dementia care, tailored to meet the unique needs of these patients.

Each ward accommodates 37-38 beds and is organized into teams comprising both registered and unregistered staff members. Our team includes apprentice healthcare assistants, international nurses, temporary staff, newcomers to healthcare, and student nurses. We pride ourselves on delivering exceptional care and support to patients and their families, especially those nearing the end of life, with the assistance of our specialist dementia palliative care nurse.

Our wards prioritize minimizing patient deconditioning during hospital stays, aided by trained exercise professionals. Currently, we are in the planning stages for a specialized Dementia Garden, designed to offer a safe and interactive outdoor space for our patients, their families, and staff.

Job Duties

  • Execute the instructions of the Registered Nurse, who is accountable for care delivery.
  • Perform duties as outlined in the Trust's Approved Core Task List for Healthcare Assistants qualified to NVQ Level 3 in Care/Health and Social Care.
  • Report any patient concerns or observed changes in clinical conditions to registered staff.
  • Accurately document all care provided to patients, ensuring confidentiality and countersigned by a Registered Nurse.
  • Respond appropriately to clinical emergencies to guarantee patients receive necessary care.
  • Adhere to all Trust policies, procedures, and nursing guidelines during activities.
  • Welcome patients to the clinical area and assist with the admission process under the supervision of registered staff.
  • Provide and deliver individualized personal nursing care in line with essence of care standards.
  • Follow Trust infection control procedures, demonstrating an understanding of cross-infection.
  • Assist in preparing patients and equipment for treatments, investigations, or procedures as directed by a Registered Nurse.
  • Support patients pre, peri, and post-procedure under the guidance of the Registered Nurse.
  • Escort patients to other clinical departments as directed by the Registered Nurse.
  • Support relatives under the guidance of a Registered Nurse.
  • Assist with the discharge process for patients as directed by the Registered Nurse.

Qualifications

Essential

  • Completion of a relevant Health Care Assistant training program, if applicable.

Desirable

  • City and Guilds NVQ Level 3 in Care/Health and Social Care.
