Community Mental Health Specialist

We are seeking a highly skilled and experienced Community Mental Health Nurse to join our team at Atlantis Medical. As a key member of our multidisciplinary care team, you will be responsible for assessing, planning, coordinating, delivering, and evaluating care for our patients.
Key Responsibilities:
- Assess patient needs and develop individualized care plans
- Communicate effectively with patients, families, and healthcare teams
- Coordinate care with other healthcare professionals and community resources
- Deliver high-quality patient care and promote health, safety, and well-being
- Participate in quality improvement initiatives and contribute to a culture of excellence
Requirements:
- Registered on the Nursing and Midwifery Council (NMC) register
- At least one year of experience at Band 5 or above in hospital or community healthcare
- Evidence of continued professional development, relevant to the role
- Clear understanding of Care Programme Approach (CPA) and care coordination
Preferred Qualifications:
- Postgraduate qualification in mental health nursing or a related field
- Experience working in a community mental health setting
- Knowledge of recovery-oriented care and trauma-informed practice
