About the Role

We are seeking an experienced and skilled Clinical Pathway Manager to join our team at Livewell Southwest. As a key member of our senior management team, you will be responsible for leading the development and implementation of our Infant and Early Help Pathways, working closely with our CAMHS team to provide safe, effective, and accessible services to children and young people.

Key Responsibilities
  • Lead the development and implementation of our Infant and Early Help Pathways, working closely with our CAMHS team.
  • Provide clinical supervision and support to our team members, ensuring they have the skills and knowledge to deliver high-quality services.
  • Work closely with our senior management team to develop and implement integrated governance requirements, ensuring compliance with CQC standards.
  • Develop and implement care plans, ensuring active involvement of service users and their carers.
  • Ensure robust systems are in place to monitor the clinical practice of team members, including risk management and incident reporting.
  • Work closely with other teams within Livewell Southwest and external agencies to ensure effective discharge planning and implementation of key success criteria.
Requirements
  • At least three years' experience in line management responsibility for one or more multi-disciplinary teams, with a focus on work with children, young people, and their families experiencing mental health problems.
  • Post-qualifying experience of clinical assessment, evaluation, care planning, and risk management in work with those experiencing mental health problems.
  • Culturally competent practice with people from a range of diverse backgrounds.
  • Experience of providing clinical supervision to professionals working with children and young people experiencing mental health problems.
  • Knowledge of the range of therapeutic skills relevant to working with children and young people aged 0-19 and their families.
  • Mental Health assessment skills and well-established case management skills.
  • Good organisational and time management skills with an ability to prioritise.
  • The ability to maintain working partnerships with other organisations in the delivery of integrated care.
  • Well-developed communication skills, both oral and written.
