The Maidenhead Primary Care Network is a collaborative of multiple GP practices dedicated to enhancing the delivery of personalized and coordinated health and social services to a diverse patient population.
We are seeking to appoint a Patient Care Facilitator to join our multidisciplinary healthcare team within the Primary Care Network.
Key Responsibilities
The ideal candidate will be instrumental in proactively identifying and assisting individuals, particularly the elderly and those with chronic conditions, to ensure effective coordination and navigation of care across various health and social services.
This role involves close collaboration with general practitioners and practice teams to guarantee that suitable support is accessible to patients, aiding them in understanding and managing their health conditions while addressing their evolving needs. The facilitator will empower individuals to access necessary services and support, ultimately enhancing their quality of life.
Additionally, the Patient Care Facilitator will collaborate with social prescribing link workers and health coaches to deliver a comprehensive approach to personalized care, guiding individuals through the complexities of the health and social care system.
The successful candidate will engage with a wide array of individuals from various cultural and social backgrounds, necessitating the ability to operate confidently and effectively in a dynamic and sometimes challenging environment.

The ideal candidate will possess exceptional interpersonal and communication skills, demonstrating organization, patience, and empathy. Experience in health, social care, or related support roles involving direct interaction with individuals, families, or caregivers is essential.
About Our Organization
The Maidenhead Primary Care Network has been successfully providing comprehensive care to the local population for several years, supported by a robust team of clinical and non-clinical professionals.
While each Patient Care Facilitator will be integrated within a specific practice, they will benefit from a collaborative approach, working alongside other members of the health and wellbeing team. The team enjoys both practice and network supervision and training opportunities.

Role Expectations
Patient Care Facilitators play a vital role within the Primary Care Network, proactively identifying and assisting individuals, including the elderly and those with long-term health issues, to ensure effective coordination and navigation of care across health and social services.
They work closely with general practitioners and practice teams to manage a patient caseload, serving as a central point of contact to ensure that appropriate support is available to patients and their caregivers. This includes helping individuals understand and manage their health conditions while addressing their changing needs.
This is achieved by consolidating all relevant information regarding a person's identified care and support needs and exploring options to meet these within a single personalized care and support plan, tailored to the individual's priorities.
Patient Care Facilitators assess patients' needs and assist them in accessing the necessary services and support to manage their health and wellbeing effectively, referring to social prescribing link workers, health coaches, and other professionals as needed.
They may also provide time and expertise to support individuals in preparing for or following up on clinical discussions with primary care professionals, enabling them to take an active role in managing their care and making informed choices.
The successful candidate will be based within a local cluster of general practices as part of the Primary Care Network. They will be caring, dedicated, reliable, and person-centered, enjoying interaction with a diverse range of individuals. Strong written and verbal communication skills, along with excellent organizational and time management abilities, are essential.
This role is designed to be an integral part of the multidisciplinary team within the Primary Care Network, working alongside social prescribing link workers and health coaches to promote and embed a personalized care approach across the network.
There may be a requirement to work remotely based on the role's needs. Please note that the position of Patient Care Facilitator is not a clinical role.
