Position Overview

We are seeking a compassionate and proactive nurse who is passionate about primary healthcare to become a vital member of our team.

Your role will involve delivering comprehensive, high-quality nursing services to our patient population.

As part of a large, collaborative multi-disciplinary team, you will significantly contribute to enhancing the health outcomes of our patients.

Key Responsibilities

Experience in General Practice is a prerequisite.

Compensation is negotiable based on your experience.

The following skills are desirable, although not exhaustive, and full-time availability is preferred:

  • Management of chronic conditions (Diabetes, COPD, Asthma, CKD, Stroke, Hypertension)
  • Provision of sexual health services
  • Family planning and women's health, including cervical screenings
  • Management of minor and complex wounds, including leg ulcers
  • Travel health consultations and vaccinations
  • Child immunization and guidance
  • Men's health assessments
  • General Practice Nurses may also oversee Health Care Assistants at the practice; mentoring skills would be advantageous.

You will be responsible for managing your own clinic and appointments daily, providing nursing care to our patients.

About Our Practice

We are a large, welcoming, and supportive practice serving a diverse patient base across multiple sites.

Our extensive team includes GPs, Advanced Clinical Practitioners, Physician Associates, Nurses, and Health Care Assistants, all supported by a dedicated team of Receptionists and Administrative staff.

We are committed to professional development and are a training practice for various clinical roles.

Job Duties

In alignment with the UKCC Code of Conduct, the successful candidate will lead and develop the nursing team in providing assessment, treatment, screening, health education services, and patient advice.

The candidate will:

  • Adhere to practice protocols and guidelines to deliver nursing treatments to patients, both independently and collaboratively with nurse colleagues and general practitioners.
  • Possess the necessary training and competence, working within patient group directives where applicable. A nurse with prescribing qualifications will utilize them appropriately.
  • Participate in regular meetings as required by the practice.

Clinical Responsibilities

Identify, assess, and diagnose patients with complex health needs across the practice population, adhering to practice policies and protocols.

  • Support the implementation of technical and specialized nursing treatments for patients with complex healthcare needs.
  • Engage in developing initiatives to enhance health and well-being, addressing the specific needs of communities, groups, and individuals.
  • Collaborate with others to modify policies aimed at improving health and well-being and reducing inequalities.
  • Work with colleagues to implement and evaluate evidence-based practices to enhance the services provided by the nursing team.
  • Lead the team in quality assurance, development, and clinical governance.
  • Collaborate with other practice team members in executing specific aspects of national frameworks.

Chronic Disease Management

  • Lead chronic disease management initiatives.
  • Identify significant abnormalities.
  • Diagnose, monitor, and develop individualized management plans, collaborating with patients and other health professionals as appropriate.

Health Screening and Immunization

  • Administer appropriate immunizations autonomously.
  • Work with patients facing complex immunization issues.
  • Support public health initiatives, ensuring the team’s work aligns with current guidelines.
  • Assist the team in achieving immunization targets.

Infection Control

Lead by example in promoting high standards of infection prevention and control throughout the practice.

Assist in developing and implementing policies and procedures related to infection control.

Provide expert advice to staff, patients, and the public regarding infection control matters.

Personal and Professional Development

Maintain continued education through participation in relevant courses and training programs to meet professional development requirements.

Engage in annual performance reviews and take responsibility for personal and professional growth.

Quality Assurance

Strive to maintain quality within the practice by:

  • Alerting team members to quality and risk issues.
  • Assessing personal performance and taking accountability for actions.
  • Contributing to team effectiveness by reflecting on activities and suggesting improvements.
  • Managing time, workload, and resources effectively.

Communication

Recognize the importance of effective communication within the team and strive to:

  • Communicate effectively with team members and patients.
  • Respond to the diverse communication needs of individuals.

Contribution to Service Implementation

Apply practice policies, standards, and guidelines in daily work and participate in audits as appropriate.
