We are seeking a highly skilled and experienced Diabetes Specialist Nurse to join our team at Calderdale and Huddersfield NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and contributing to the development of our diabetes services.
Main Responsibilities- Provide specialist nursing care to patients with diabetes, including those with complex needs, in an in-patient setting.
- Lead the nursing team in the delivery of diabetes care, working closely with medical staff and other healthcare professionals to ensure seamless patient care.
- Develop and maintain a high level of specialist knowledge and skills in diabetes care, staying up-to-date with the latest research and evidence-based practice.
- Act as a role model for other healthcare professionals, promoting excellence in patient care and contributing to the development of best practice guidelines and protocols.
- Participate in research and evaluation of our diabetes services, ensuring that we provide the highest quality care to our patients.
- Deliver direct specialist nursing care to patients with diabetes, including those with complex needs, in an in-patient setting.
- Initiate oral and injectable therapies within your scope of competency, working closely with medical staff to ensure safe and effective patient care.
- Run specialist clinics and manage a caseload of patients with type 1 and type 2 diabetes, providing high-quality care and support.
- Promote and facilitate research and evidence-based practice in diabetes nursing, contributing to the development of best practice guidelines and protocols.
- Support the establishment, implementation, and monitoring of nursing practice guidelines, standards, policies, and protocols/care pathways.
- Act as a patient advocate at all points of the care pathway, ensuring that patients receive high-quality care and support.
- Adhere to the Trust's policy and procedure, ensuring that patient care is delivered in a safe and effective manner.
- Act always in accordance with the NMC code of conduct and guiding documents, upholding the highest standards of professional practice.
- RGN qualification and post-registration experience in diabetes care.
- Teaching qualification and post-graduate qualification in a relevant field.
- Ability to demonstrate experience of audit and research, including implementation of findings and outcome reviews.
- Experience of working within a multidisciplinary team, with a strong focus on patient-centered care.
- Ability to communicate verbally and in writing, with a strong focus on patient education and empowerment.
