We are seeking a dedicated and compassionate Care and Support Professional to join our team at Albion House in Driffield. As a Support Worker, you will play a vital role in supporting our customers to live their best lives, promoting their independence, and helping them achieve their goals and aspirations.
Key Responsibilities
- Assist customers with daily living tasks, such as making meals, shopping, and budgeting, to develop their independence.
- Support customers with their personal care and medication needs, as outlined in their support plans.
- Carry out risk assessments, support planning, goal setting, and regular reviews to ensure the best possible outcomes for our customers.
- Work collaboratively with colleagues to provide a supportive and inclusive environment for our customers.
- Use technology to create and update support plans, complete mandatory learning, and stay in touch with colleagues.
