Position Overview

East Sussex Healthcare NHS Trust is actively expanding its Acute Therapy and Community Response services as part of a transformative initiative aimed at implementing a Home-First model, which emphasizes delivering care within the community.

This enhanced service will function across the integrated pathway, bridging acute and intermediate care services in the community, assisting patients with their rehabilitation and care needs, thereby improving clinical outcomes and patient satisfaction. We are committed to investing in both therapy and care roles, and successful candidates who join our expanded service will have the opportunity to be part of a dynamic, progressive, integrated, multi-agency, and multidisciplinary team dedicated to providing comprehensive care to the residents of East Sussex.

As a team member, you will work closely with and receive support from our Professional Leads, Advanced Clinical Practitioners, Team Leads, Practice Educators, and the wider team, enabling you to enhance your skills, training, education, and career development, whether you are newly qualified or aspiring to a senior role.

Key Responsibilities

This position involves community-based responsibilities; therefore, you must be able to travel efficiently throughout the area, carrying necessary equipment and staff. A vehicle and a valid driving license are essential.

Job Qualifications

Necessary

  • Good level of general education, including English and Maths
  • National Vocational Qualification (NVQ) Level 3 or equivalent practical experience
  • Satisfactory General Education and evidence of suitable additional training related to the role

Preferable

  • Educated to GCSE level

Work Experience

Necessary

  • Prior experience in nursing or social care settings
  • Customer service experience
  • Experience in working independently and in a team environment

Preferable

  • Previous experience in community nursing and/or Physiotherapy, Occupational Therapy
  • Experience working in a community setting

Knowledge Requirements

Necessary

  • Fundamental understanding of the NHS and healthcare system
  • Knowledge of basic infection control principles
