About the Role

We are seeking a highly skilled and compassionate Support Assistant to join our team at Home Care Direct. As a Support Assistant, you will play a vital role in providing personalized care and support to our clients, enabling them to live independently and safely in their own homes.

Key Responsibilities
  • Provide personal care and support to clients with complex needs, including assistance with daily living activities, mobility, and communication.
  • Develop and maintain strong relationships with clients, their families, and other healthcare professionals to ensure seamless care delivery.
  • Work collaboratively with our multidisciplinary team to identify and address client needs, develop care plans, and implement interventions.
  • Conduct thorough assessments of client needs, identifying areas for improvement and developing strategies to address these.
  • Provide training and education to clients and their families on various aspects of care and support.
  • Participate in regular team meetings and contribute to the development of care plans and policies.
Requirements
  • Experience of working with clients with Learning Disabilities and Autism.
  • Ability to work flexibly, including evenings, weekends, and bank holidays.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with clients, families, and colleagues.
  • Ability to work independently and as part of a team, with a high level of autonomy and initiative.
  • Strong problem-solving and analytical skills, with the ability to think critically and creatively.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
