Position Overview

The Clinical Pharmacy Support Specialist will collaborate with Clinical Pharmacists to implement the Clinical Pharmacy model of care within the Primary Care Network (PCN). This role aims to enhance safe and cost-effective prescribing practices to elevate patient care quality. The specialist will assist patients in optimizing their medication regimens by adhering to established protocols, refining repeat prescribing workflows, and promoting online ordering while minimizing clinical risks and reducing medication waste.

Key Responsibilities

The Clinical Pharmacy Support Specialist will play a vital role in assisting the Clinical Pharmacist with the execution of the Clinical Pharmacy model. The successful candidate will:

  • Conduct medication switches in GP practices, ensuring accurate patient record updates.
  • Perform general maintenance tasks within patient records, including dose optimization and medication synchronization.
  • Enhance prescribing processes across general practice settings.
  • Encourage the use of electronic repeat dispensing and online ordering systems.
  • Report on key performance indicators to demonstrate the value added to patients and practices in line with national guidelines.
  • Utilize and maintain relevant information systems and databases for incident reporting, medication optimization, and tracking patient outcomes.
  • Collaborate closely with GP practices and the ICB Clinical Pharmacy lead.
  • Engage in practice meetings and patient participation groups to foster understanding of the pharmacy technician role and address prescribing issues.
  • Coordinate with other pharmacy stakeholders, including community and hospital pharmacy teams.
  • Support the Medicines Optimization team in executing the Medicines Optimization strategy.
  • Handle sensitive information in compliance with data protection regulations.
  • Organize and manage personal workload to meet established deadlines.
  • Conduct prescribing audits using GP practice clinical systems.
  • Participate in clinical audits to facilitate improvements in practice.
  • Monitor medication alerts and advise on necessary changes.
  • Assist in tracking prescribing expenditures and suggest adjustments to stay within budget.
  • Engage in ongoing professional development to maintain necessary skills and knowledge.
  • Perform additional duties as required to meet service priorities.

Qualifications and Experience

Essential:

  • Professional registration with GPhC.
  • BTEC/NVQ level 3 or equivalent in pharmaceutical sciences.
  • Demonstrated experience as a registered pharmacy technician in a primary care, community, or hospital setting.
  • Evidence of continued professional development.

Desirable:

  • Postgraduate qualifications in a relevant field.
  • Knowledge of current healthcare policies and their implications for clinical pharmacy.

Skills and Knowledge

Essential:

  • Advanced knowledge of general practice and evidence-based medicine.
  • Understanding of patient management systems in primary care.
  • Strong clinical pharmacy knowledge and terminology.
  • Ability to analyze and interpret prescribing data effectively.
  • Excellent communication skills, both verbal and written.
  • Proficiency in Microsoft Office applications.

Desirable:

  • Familiarity with GP clinical systems.
  • Attention to detail and strong organizational skills.
