Job Overview

The Clinical Pharmacy Support Specialist will collaborate with Clinical Pharmacists to implement the Clinical Pharmacy model of care within the Primary Care Network (PCN). This role is pivotal in enhancing safe and cost-effective prescribing practices to elevate patient care quality. The specialist will assist patients in optimizing their medication regimens by adhering to established protocols, refining repeat prescribing workflows, and promoting online ordering and repeat dispensing, all while minimizing clinical risks and reducing medication waste.

Key Responsibilities

The Clinical Pharmacy Support Specialist will play a crucial role in assisting the Clinical Pharmacist with the deployment of the Clinical Pharmacy model in General Practice. The selected candidate will be responsible for executing medication switches in GP practices, which includes reviewing and updating patient records. Additionally, they will perform essential tasks such as dose optimization and medication synchronization to enhance the prescribing process. A professional registration with GPhC and a BTEC/NVQ level 3 or equivalent in pharmaceutical sciences are required, along with a proven track record of delivering professional expertise.

Coastal PCN comprises three practices that have united to provide enhanced services under the National DES, serving a community of over 39,000 registered patients. Our diverse team includes Social Prescribers, Care Coordinators, Pharmacists, Pharmacy Technicians, Home Visiting Service professionals, Physiotherapists, Mental Health Practitioners, and Health and Wellbeing Coaches.

Additional Responsibilities

- Support the Clinical Pharmacist in implementing the Clinical Pharmacy model at the PCN level.

- Conduct medication switches in accordance with agreed protocols, updating patient records as necessary.

- Perform housekeeping tasks within patient records, including dose optimization and medication synchronization.

- Enhance prescribing processes within general practice.

- Promote electronic repeat dispensing and online ordering.

- Report on key performance indicators to demonstrate the value added for patients and general practice, in line with NICE guidance.

- Utilize and maintain relevant information systems and databases, including incident reporting and prescribing data.

- Collaborate closely with GP practices and the ICB Clinical Pharmacy lead.

- Engage in practice meetings and patient participation groups to enhance the role of pharmacy technicians within the Clinical Pharmacy model.

- Liaise with pharmacy stakeholders, including the ICB Medicines Optimisation team and community pharmacy.

- Assist in the delivery of the Medicines Optimisation strategy.

- Handle sensitive and complex information in compliance with the Data Protection Act and confidentiality guidelines.

- Organize and prioritize workload to meet deadlines.

- Conduct prescribing audits using GP practice clinical systems.

- Participate in clinical audits to facilitate improvements in practice.

- Ensure timely action on major MHRA drug alerts and advise on medication changes.

- Monitor prescribing expenditure and propose changes to maintain budgetary compliance.

- Engage in continuous learning and development to maintain necessary knowledge and skills.

- Undertake additional duties as required, aligned with the skills and competencies of the role.

Qualifications and Experience

Essential

  • Professional registration with GPhC.
  • BTEC/NVQ level 3 or equivalent in pharmaceutical sciences.
  • Evidence of continued professional development.
  • Experience as a registered pharmacy technician in primary care, community, or hospital settings.

Desirable

  • Postgraduate qualifications in a relevant field.
  • BTEC Therapeutics.

Knowledge and Skills

Essential

  • Advanced knowledge of general practice and evidence-based medicine.
  • Understanding of systems for managing patients in primary care.
  • Strong clinical pharmacy knowledge.
  • Awareness of pharmacy law, ethics, and current legislation.
  • Ability to analyze and interpret prescribing data.
  • Excellent communication skills, both verbal and written.
  • Proficiency in Microsoft Office.

Desirable

  • Familiarity with GP Clinical Systems.
