The successful candidate will deliver expert assistance to individuals facing cancer and other terminal conditions. This role is vital within the healthcare team at the Trust, serving as the primary contact for patients and all professionals seeking access to the Specialist Palliative Care services. The individual will perform various responsibilities to ensure high-quality care delivery, including providing specialized guidance and continuous support to the designated patient group.
A tailored professional development plan will be created to facilitate the candidate's growth, utilizing a recognized competency framework that allows the individual to excel in all aspects of the role. This will foster advancement in areas such as leadership, education, and the ability to influence the strategic direction of palliative care within the Trust.
The candidate will receive mentorship and support from Band 7 specialist nurses while managing a supervised patient caseload.
Key Responsibilities
The individual will maintain established connections between the acute care sector and primary healthcare teams to ensure seamless patient transitions between hospital and community settings. This will involve collaboration with services within the South Tees Hospitals NHS Foundation Trust and other essential partners.
Although primarily based in a hospital environment, the candidate will be expected to rotate across the service, including both acute and community settings, as necessary. Participation in the 7-day service is required, which includes working weekends and bank holidays on a rotational basis.

Attendance at the leadership development program and the Quality Improvement Programme is expected. This initiative aims to:

Upon completion of this four-and-a-half-day training, participants will have the opportunity to pursue additional leadership and improvement training, including in-house courses tailored to their roles, as well as bite-sized programs and leadership apprenticeships at levels 3, 5, and 7.
