Job Title: Patient Care Navigator

Job Summary:

The Patient Care Navigator will serve as the primary point of contact for patients, providing exceptional customer service and arranging appointments with the most suitable clinician to meet their needs. They will also offer guidance on available services and options.

This role will be based in the central call centre of The Lewisham Care Partnership and at practice sites for face-to-face services. The successful candidate will be expected to work in both settings.

Main Responsibilities:
  • Process and effectively signpost patients to the appropriate healthcare professional based on their presenting condition.
  • Answer incoming phone calls, transferring calls or addressing caller requests appropriately.
  • Process patient requests for appointments.
  • Process repeat prescription requests.
  • Initiate contact with and respond to requests from patients, team members, and external agencies.
  • Enter read-code data on the Emis Web clinical system.
  • Photocopy documentation as required.
  • Data entry of new and temporary registrations and relevant patient information as required.
  • Input data into patients' healthcare records as necessary.
  • Direct requests for information, SAR, insurance/solicitors letters, and DVLA forms to the administrative team.
  • Manage all queries efficiently.
  • Carry out system searches as requested.
  • Maintain a clean, tidy, and effective working area at all times.
  • Monitor and maintain the reception area and noticeboards.
  • Support all clinical staff with general tasks as requested.
