Primary Care Careers is excited to announce a new opportunity for a Healthcare Coordination Specialist. This full-time position involves working collaboratively across various local healthcare practices to support a diverse patient population. Key Responsibilities

  • Proactively identify and engage with a designated group of individuals to address their personalized care needs.
  • Consolidate the identified care and support requirements of individuals and develop a comprehensive personalized care plan.
  • Promote awareness of shared decision-making practices and assist individuals in preparing for collaborative discussions regarding their care.
  • Provide navigation and coordination support for individuals and their caregivers across various health and social care services, while collaborating closely with social prescribing link workers, health coaches, and other primary care professionals.
  • Facilitate and support the organization of multi-disciplinary team meetings (MDTs) within the network.
  • Document meeting minutes and ensure that clear actions and coordinated responses are recorded effectively.

Qualifications

Essential

  • ECDL or equivalent certification
  • Diploma/HNC Level or relevant experience in a related field
  • NVQ Level 2 in Business Administration or equivalent experience

Experience

  • Demonstrated commitment to ongoing professional and personal development
  • Advanced proficiency in Microsoft Word, Excel, and PowerPoint, with the ability to create documents, manage emails, and utilize the internet for generating reports
  • Capability to synthesize information, maintain accurate records, and prioritize tasks effectively
  • Creative problem-solving skills, with a willingness to seek out challenging information
  • Ability to analyze large volumes of information and extract key insights for reporting purposes
  • Experience in data collection and providing monitoring information to evaluate service effectiveness
  • Experience in collaborative partnership working
  • Strong organizational skills, with the ability to plan and prioritize independently, even under pressure
  • Ability to work effectively within a team environment
  • Proficient in supporting individuals in a manner that fosters trust and confidence
  • Excellent communication skills, both verbal and written, with the ability to engage effectively with patients, families, caregivers, partner organizations, and other stakeholders
