Job Summary

The successful candidate will be responsible for the enhanced hospital management of homeless individuals referred to the team or identified by the team.

They will work in partnership with other members of their own multi-disciplinary team, as well as other hospital teams and external agencies to achieve successful discharges.

The post holder will provide specialist discharge advice and support to other healthcare professionals in the hospital as necessary.

They will work across wards, in Accident and Emergency, and may also outreach into the community to support patients post-discharge.

Main Responsibilities
  • Provide in-depth knowledge of homeless health services that can be referred to on discharge.
  • Engage in significant partnership working aimed at improving hospital discharge for individuals and groups of patients.
  • Provide direct support to medical and surgical teams, therapists, and ward sisters in discharging patients in a safe and timely manner.

Person Specification

The successful candidate will have:

  • Working experience of inclusion health and experience working in a specialty involved with complex clients with problems such as homelessness or social exclusion.
  • Current Level One Nursing registration and educated to master level or able to demonstrate equivalent experience.
  • Knowledge of local resources and understanding and knowledge of the complex needs of individuals and families who are homeless or socially excluded.
