About the Role

We are seeking a highly skilled and experienced Senior Community Children's Nurse to join our team at Homerton University Hospital. As a key member of our Community Children's Nursing team, you will play a vital role in providing high-quality, patient-centered care to children and their families in the community.

Key Responsibilities
  • Provide specialist nursing care to children in the community, working closely with parents and carers to ensure the best possible outcomes.
  • Develop and implement individualized care plans, taking into account the unique needs and circumstances of each child and family.
  • Work collaboratively with other healthcare professionals to ensure seamless care transitions and effective communication.
  • Contribute to the development and implementation of policies and procedures related to community children's nursing, ensuring that they are evidence-based and aligned with best practice.
  • Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of solutions.
Requirements
  • Registered Children's Nurse with a strong background in community children's nursing.
  • Proven experience of working with children and families in the community, with a focus on providing high-quality, patient-centered care.
  • Excellent communication and interpersonal skills, with the ability to work effectively with colleagues, families, and other healthcare professionals.
  • Ability to work autonomously, with a high degree of independence and self-motivation.
  • Strong problem-solving and critical thinking skills, with the ability to analyze complex situations and develop effective solutions.
