Position Overview

Join our dynamic and award-winning Urgent Community Response (UCR) Service at Stockport NHS Foundation Trust, where we are expanding our capabilities to include virtual ward support while enhancing our established UCR model.

The UCR services are dedicated to assisting patients within their homes throughout the Stockport community, aiming to prevent hospital admissions, facilitate safe discharges, and provide virtual ward care through innovative remote monitoring technologies and comprehensive medical oversight.

We are seeking dedicated and passionate individuals for the role of Senior Support Worker. If you are a motivated professional eager to contribute to a team delivering exceptional urgent community care, we encourage you to consider this opportunity.

Key Responsibilities

Your primary focus will be on delivering personal, health, and therapeutic care to patients with diverse and acute needs. Your daily activities will be varied and engaging, aimed at promoting patient independence in daily living activities. You will collaborate with nursing, physiotherapy, and occupational therapy professionals to assess patient needs and facilitate referrals.

Your responsibilities will include:

  • Assisting in the ongoing evaluation and delivery of care, rehabilitation, and documenting any changes in patient conditions to your registered colleagues, both verbally and in written form.
  • Performing basic clinical and therapeutic tasks such as taking vital signs, administering medications, managing simple wound care, and implementing rehabilitation programs.
  • Recording interactions with patients, their families, and caregivers in the electronic patient record system.
  • Empowering patients to regain their health and independence in partnership with the multidisciplinary team.
