Job Overview

We are seeking a dedicated Medication Management Technician to become a vital part of our expanding multidisciplinary team at Lakeside Healthcare Group.

This position is integral to the transformation of pharmacy services within General Practice, aligned with the NHS Long Term Plan.

The successful candidate will collaborate with the Lakeside Healthcare Pharmacy Team to promote the safe and effective use of medications for our patients, adapting to the evolving needs of our healthcare network.

Key Responsibilities

The role involves working closely with the Lakeside Healthcare Pharmacy team to ensure optimal medication management for patients.

Responsibilities include:

  • Conducting medication reconciliation from clinical documentation.
  • Playing a central role in shared care protocols and ensuring compliance with the ICB Formulary.
  • Performing medication reviews.
  • Assisting Clinical Pharmacists with routine prescription inquiries and medication-related questions.
  • Engaging in practice meetings to enhance the role of the Medication Management Technician and advocate for key issues related to antimicrobial stewardship and prescribing quality.
  • Participating in ongoing training and development to maintain the necessary skills and knowledge for practice-based responsibilities.
  • Supporting the implementation of medicines optimization initiatives and audits related to patient safety and high-risk drug monitoring.
  • Building relationships with other pharmacy professionals and multidisciplinary team members to facilitate the integration of pharmacy services across various healthcare settings.

Experience and Qualifications

Essential

  • Proven commitment to professional development.
  • Experience as a Medication Management Technician in a community pharmacy setting.

Desirable

  • Experience in a clinical commissioning group or hospital pharmacy.
  • Familiarity with clinical systems such as System One.

Qualifications

Essential

  • BTEC/NVQ level 3 or equivalent in pharmaceutical sciences.
  • Professional Registration with the GPhC.
