About the Role

We are seeking a highly motivated and empathetic Community Support Worker to join our multi-disciplinary team at Lincolnshire Partnership NHS Foundation Trust. As a Community Support Worker, you will play a vital role in providing support and care to individuals with mental health needs in the community.

Key Responsibilities
  • Work collaboratively with the Community Psychiatric Nurse/Social Worker/Occupational Therapist to provide high-quality care and support to individuals with mental health needs.
  • Develop and maintain strong relationships with service users, their families, and carers to ensure a person-centered approach to care.
  • Conduct risk assessments and develop individualized care plans to promote independence and well-being.
  • Provide emotional support and guidance to service users, promoting their physical and mental health.
  • Work flexibly across a 7-day-a-week service, including evenings and weekends, to meet the needs of service users.
  • Participate in regular clinical supervision sessions to ensure high-quality care and support.
  • Collaborate with other healthcare professionals to ensure seamless care and support.
Person Specification

We are looking for a candidate with:

  • A Cavendish Care Certificate or willingness to work towards it, resourced by LPFT.
  • NVQ 3 in Care or equivalent.
  • NVQ 3 in Promoting Independence or equivalent or working towards achievement.
  • Training in mental health awareness or other relevant training.
