The primary objective of this assessment area is to evaluate patients transferred from emergency departments, ambulance services, and general practitioners, while conducting a thorough geriatric assessment. This process enables the multidisciplinary team to adopt a comprehensive approach to patient care, ensuring safe discharges into community care pathways unless an acute admission is warranted.

Detailed Job Description and Main Responsibilities

Following the completion of pre-employment checks, candidates will be contacted by the recruitment team to arrange a start date aligned with our corporate induction schedule.

After the corporate induction, new hires will participate in a comprehensive 2-week induction program tailored for various healthcare roles. This program is full-time and may involve travel between Trust sites, incorporating both clinical shifts and classroom-based sessions, along with e-learning components.

Please be aware that appointment to this position is contingent upon a satisfactory Enhanced Disclosure & Barring Service Check, which includes regulated checks against vulnerable adults and children.
