About the Role:
We are seeking a skilled and compassionate Complex Needs Practitioner to join our team at Recovery Focus. As a key member of our multi-disciplinary team, you will work with individuals and families affected by substance misuse, providing holistic support and empowering them to achieve positive outcomes.
Key Responsibilities:
- Engage with individuals and families affected by substance misuse, using a person-centred approach to empower them to reduce risk and increase resilience.
- Work collaboratively with our multi-agency partnership settings, including health and social care professionals, to provide comprehensive support.
- Develop and implement tailored support plans, addressing the holistic needs of individuals and families, including physical and emotional wellbeing, relationships, and meaningful activity.
- Provide psychosocial interventions, using recognised best practice family interventions, in collaboration with our partners ADFAM.
- Work flexibly across the community, with a central hub base at our head office, to provide outreach support and engage with clients in various settings.
Requirements:
- Experience in Family/Carer work or a qualification in health/social care, youth and community work (e.g. NVQ Level 3 or above, DipSW, Mental Health Nursing, Counselling, Addiction Studies).
- Experience of liaising with voluntary and statutory agencies and health professionals, and engaging effectively with clients in a variety of settings.
- Excellent record keeping and report writing skills, with a willingness to work flexibly and adapt to changing circumstances.
- A commitment to increasing diversity and a willingness to work with individuals from diverse backgrounds.
