Position Overview

This role presents a unique opportunity for a Junior Clinical Fellow (post Foundation) to engage with the supportive Acute Medicine department at Whittington Health NHS Trust, including the ambulatory care unit. It is an excellent position for a medical professional contemplating a career in Medicine or seeking to enhance their experience across various competencies, including procedural skills. You will receive guidance in preparing for examinations and pursuing your professional aspirations.

Each position rotates between the ambulatory care unit and the primary Acute Medicine unit (which encompasses the Acute Medicine wards, General Internal Medicine take, and post-take shifts) every three months. The AMU schedule is a full shift rotation with night and on-call duties, while the ambulatory care schedule consists of day shifts only, including one in five weekends.

Your clinical and educational supervisor will remain consistent throughout the year, as the consultants and multidisciplinary team collaborate across departments. This rotational structure allows you to concentrate on diverse skills and personal growth during each assignment.

You will collaborate with a dynamic multidisciplinary team. Your training will be integrated with that provided to doctors in training programs within the department, and you will be entitled to study leave and an e-portfolio (HORUS). You will be encouraged to develop as a clinician and engage in governance activities, including mortality reviews, Quality Improvement Projects (QIPs), and departmental as well as trust-wide educational initiatives.

Detailed Job Description and Main Responsibilities

Duties of the Position

During your 'AAU' assignment: You will engage across the Acute Medical wards, medical take (day/night shifts), and Ambulatory care shifts (full shift 14-week rotation).

During your 'AEC' assignment: You will work exclusively in Ambulatory Care (day shifts/1 in 5 weekend day shifts). During your AEC assignment, you will have the option to undertake a 10% (averaging half a day per week) Teaching Fellow role for the Undergraduate Medical Students assigned to the unit.

Acute Assessment Unit (AAU)

This is a 34-bed unit situated across two adjacent wards. It is staffed daily by 2 Consultants, a registrar, 2-4 SHOs, and 2-3 FY1s, with daily Consultant-led ward rounds.

The broader AMU staffing includes a dedicated and highly trained nursing, physiotherapy, occupational therapy, and multidisciplinary team, along with a frailty team, pharmacy, and a full complement of support staff.

Facilities include full cardiac monitoring and isolation capabilities, catering to a varied patient demographic, including those with infectious diseases, patients requiring non-invasive ventilation or high-flow nasal oxygen, complex diagnostic challenges, and acute oncology cases. As a London-based District General Hospital, we admit all patients under the Acute Medicine team and maintain close collaborations with ITU/CCOT, local medical specialties, and tertiary centers to deliver exceptional patient-centered care.

Ambulatory Emergency Care (AEC)

The award-winning Ambulatory Emergency Care Unit is staffed by a dedicated team of senior and junior doctors and Advanced Clinical Practitioners who manage acutely unwell patients in an outpatient same-day emergency care setting, providing an alternative to hospital admission for many patients.

The ambulatory care unit has a dedicated Consultant of the day who oversees the diagnosis, management, and treatment of patients in the unit and provides teaching and support for various procedures.

Teaching Fellowship Opportunity

For 2 of the 4 positions (to be determined at interview), you will have the option to take up a 10% Teaching Fellowship, which will comprise approximately half a day per week during your AEC assignment. This will be supervised by the Acute Medicine Undergraduate Lead and will primarily involve delivering the teaching timetable for medical students rotating within the department, including their procedural and case-based training.

Acute Medical Take

This is staffed by 1 SpR, 2 SHOs, and 1 FY1. The unit experiences an average intake of 30 patients over 24 hours. During the day, there is a 'hot' post-take consultant ward round providing immediate feedback and learning opportunities. Overnight, you will be supervised by a medical registrar on-site and a General Internal Medicine consultant by phone. A strong and supportive Critical Care Outreach Team (CCOT) is available alongside the ITU team. As a District General Hospital, most tertiary services are coordinated through liaison with other London centers.
