About the Role
Achieving for Children is seeking a skilled Social Worker to join our Children with Disabilities Team. As a Social Worker, you will play a vital role in supporting children and young people with significant and profound disabilities.
Key Responsibilities
- Assess the needs of children and young people with disabilities and develop plans to meet their needs.
- Work closely with families, health professionals, and other agencies to ensure children receive the support they need.
- Conduct home visits, school visits, and community visits to assess children's needs and provide support.
- Develop and implement care plans to ensure children's needs are met.
- Work collaboratively with other professionals to ensure children receive a coordinated service.
About You
We are looking for a qualified Social Worker with experience working with children with disabilities. You will have excellent communication and interpersonal skills, and be able to work effectively with families and other professionals.
