Role Responsibilities:

As a valued member of our NHS Continuing Healthcare Team, you will engage with healthcare professionals to assist individuals in navigating their care pathways after a hospital discharge. Your responsibilities will include conducting assessments in accordance with the Care Act 2014 and for NHS Continuing Healthcare eligibility, collaborating closely with clients and their families to ensure a smooth transition to suitable care options.
