Job Summary:

We are seeking a highly skilled and dedicated Social Worker to join our NHS Continuing Healthcare Team at Hampshire County Council. As a qualified Social Worker, you will work collaboratively with Health colleagues to support individuals with their onward care journey following a Discharge to Assess period from hospital.

Key Responsibilities:

  • Undertake assessments under the Care Act 2014 and for NHS CHC to ensure individuals receive appropriate care provision.
  • Work in partnership with individuals and their families to develop and implement care plans.
  • Collaborate with colleagues to achieve successful outcomes within a dynamic multi-disciplinary environment.

Requirements:

  • UK-based Adults' Social Work experience with relevant professional registration.
  • Strong analytical and problem-solving skills, with the ability to work in a fast-paced environment.
  • Excellent communication and interpersonal skills, with the ability to build strong relationships with colleagues and service users.

Travel Requirements:

As a Social Worker, you will be required to travel to various locations within Hampshire to attend face-to-face assessments, meetings, and training. A current driver's license and access to a reliable vehicle are essential for this role.
