About the Role:

This is an exciting opportunity for an experienced nurse to join our busy and supportive community team at Sue Ryder. If you are a dedicated and compassionate Senior Registered Nurse who is passionate about delivering high-quality palliative care, we would like to hear from you

Key Responsibilities:

  • Provide palliative care support and advice to patients, their families, and other care professionals through a 24-hour telephone support line.
  • Work alongside Nurses and Healthcare Assistants to deliver individualized care to patients on the Palliative Care Hub register.
  • Assess, plan, refer, implement, and evaluate care plans to ensure patients receive high standards of care.
  • Support and lead some of the HCAs and be responsible for the allocation of the team's visits.

About You:

  • NMC registered with post-registration experience in palliative and end-of-life care.
  • Ability to be flexible, positive, and work within the wider team.
  • Excellent soft communication skills.
  • Ability to lead and empower others.
  • Knowledge of SystemOne and the independent sector is advantageous.
