We are seeking a highly motivated and compassionate Community Outreach Support Worker to join our team at Longhurst Group. As a key member of our Floating Support team, you will play a vital role in providing a comprehensive range of support services to customers with profound and enduring Mental Health issues and customers with Learning Disabilities, alongside those with a dual diagnosis.
Key Responsibilities- Maximising Independence: Support customers to lead independent lives, making real choices and decisions about their care and support.
- Community Engagement: Assist customers to engage and participate in their local community, accessing opportunities and services that promote their well-being.
- Economic Wellbeing: Provide support to ensure customers have access to their benefits entitlements, manage their finances, and maintain economic stability.
- Independent Living: Assist customers to live independently, maintaining their own homes and managing their daily lives.
- Record Keeping: Maintain accurate records of support provided, risk assessments, and support plans.
- Collaboration: Work closely with partner agencies, such as Birmingham City Council and localised NHS services, to ensure seamless support for customers.
- Experience: Ideally, you will have experience working in a similar role supporting individuals with enduring mental health needs.
- Knowledge: You will have a good understanding of Mental Health and Learning Disabilities, including the issues surrounding these conditions.
- Skills: You will possess excellent communication and listening skills, with the ability to provide advice and support to customers.
- Driver's Licence: You will hold a full driver's licence and have access to a vehicle to travel to customers across Birmingham.
