Key Responsibilities
  • Assess, plan, implement, and evaluate treatment and collaborative care planning for service users.
  • Provide cognitive testing and assessment in the community or hub base, using evidence-based tools and techniques.
  • Deliver group and one-to-one education interventions to promote healthy aging and dementia care.
  • Maintain and promote a personalized approach in all aspects of service delivery, ensuring that service users and their significant others are at the center of care.
  • Work collaboratively with peers to coordinate the duties and responsibilities of other nursing and support staff within the service.
  • Actively contribute to the development and delivery of therapeutic and treatment plans, and psychosocial and educational interventions.
  • Ensure that each allocated service user has a comprehensive, clear, and up-to-date collaborative care plan with specified objectives, designed to meet their identified care needs.
Person Specification

We are looking for a professional with:

  • Registered Nurse Mental Health or Occupational Therapist qualification, with at least 2 years post-grad experience.
  • Working knowledge and understanding of older adults with organic mental health conditions.
  • Leadership and interpersonal qualities, effective time management, and the ability to demonstrate sound knowledge and application of up to date evidence-based clinical practice in dementia care.
