Care and Community Services Officer

We're seeking a compassionate and experienced individual to join our team as a Community Support Worker. As a key member of our team, you'll play a vital role in supporting vision-impaired veterans in Nottinghamshire, South Yorkshire, Lincolnshire, and Derbyshire to achieve their goals and maximize their independence.
About the Role
As a Community Support Worker, you'll be responsible for carrying out risk assessments, identifying the needs of individuals, and responding to queries and feedback to better coordinate services. You'll work collaboratively with our members to plan and oversee the delivery of specialist interventions, wellbeing programs, rehabilitation events, and other activities that promote their independence and wellbeing.
Key Responsibilities
- Act as the service coordinator and main point of contact for members
- Demonstrate and issue equipment
- Support the development and work of our volunteers
- Support our partnerships for joint working, member support, and future programs
- Ensure compliance with safeguarding and risk management policies
About You
To be considered for this role, you'll need:
- Experience working with people with disabilities to maximize their independence and wellbeing
- Experience and knowledge of completing comprehensive, holistic assessments
- Experience delivering remote and face-to-face services
- The ability to use assessment tools and action plans
- The ability to identify safeguarding concerns and act in line with policies
- A relevant Level 4 qualification or equivalent experience
