About the Role

We are seeking a highly skilled and dedicated Family Health Practitioner to join our Emotional Health and Resilience Team. As a key member of our team, you will work closely with children, young people, parents, carers, and families to provide high-quality services and support.

Main Responsibilities

  • Work as part of the children's community health commissioned service under the direction and supervision of a Band 6 Caseload Holder.
  • Contribute to the provision of high-quality services for children, young people, parents, carers, and families.
  • Deliver the national and Public Health agendas in response to identified health and social need.
  • Work in partnership with other agencies to meet agreed standards, objectives, and statutory requirements.
