About the Role

We are seeking a highly skilled and dedicated Family Health Practitioner to join our Emotional Health and Resilience Team. As a key member of our team, you will work closely with children, young people, parents, carers, and families to provide high-quality services and support.

Main Responsibilities

  • Work as part of the children's community health commissioned service under the direction and supervision of a Band 6 Caseload Holder.
  • Contribute to the provision of high-quality services for children, young people, parents, carers, and families, working within the framework of the Healthy Child Programme and Best Start In Life priorities.
  • Be proactive in delivering the national and Public Health agendas in response to identified health and social need, working in partnership with other agencies to meet agreed standards, objectives, and statutory requirements.
