Job Summary

We are seeking a highly skilled and experienced Senior Mental Health Practitioner to join our team at Cambridgeshire and Peterborough NHS Foundation Trust. As a key member of our multidisciplinary team, you will be responsible for providing holistic psychosocial assessments, developing and implementing individual packages of care and treatment, and promoting a recovery model that empowers patients, carers, and relatives.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify individual needs and develop personalized care plans.
  • Provide brief care and treatment interventions to support patients in achieving their goals and improving their mental health.
  • Collaborate with referrers, healthcare professionals, and other stakeholders to ensure seamless care coordination and continuity.
  • Participate in the development and implementation of new initiatives to improve clinical care and patient outcomes.
  • Contribute to the training and support of students and junior staff members.
Requirements
  • Current registration with the relevant professional body (NMC, HCPC, or Social Work Registration).
  • Post-qualification specialist mental health training and experience in a community setting.
  • Ability to deliver mental health awareness and education to patients, carers, and staff.
  • Working knowledge of the Mental Health Act and relevant policies, including SOVA and Safeguarding.
  • Ability to travel across a large geographical area and work flexible hours, including evenings and weekends.
