Job Title: Adult Mental Health Practitioner

Cambridgeshire and Peterborough NHS Foundation Trust is seeking a skilled Adult Mental Health Practitioner to join our team.

About the Role

We are looking for a dedicated and compassionate individual to provide holistic psychosocial assessments, develop and implement individual packages of care and treatment, and undertake clinical risk assessments. The successful candidate will be responsible for promoting a recovery model that empowers patients, carers, and relatives to take ownership of their care and treatment.

Key Responsibilities
  • Provide holistic psychosocial assessments and develop individual packages of care and treatment
  • Undertake clinical risk assessments and manage risk
  • Develop and implement brief care and treatment interventions for individual patients
  • Promote a recovery model that empowers patients, carers, and relatives
  • Liaise effectively with referrers and professionals within the trust
  • Participate as an active member of the multi-disciplinary team
Requirements
  • Current NMC/HCPC/Social work Registration
  • Post qualification specialist mental health training
  • Experience of working in a community setting
  • Ability to deliver mental health awareness and varied concise information
  • Ability to develop and lead new initiatives within the clinical care environment
