About the Role

We are seeking a highly skilled and experienced Mental Health Liaison Practitioner to join our team at Livewell Southwest. As a key member of our CAMHS Eating Disorder Pathway, you will play a vital role in supporting the admission process for young people with eating disorders to Derriford Hospital.

Your primary responsibility will be to oversee and support the admission process, ensuring that young people receive the highest level of care and support. You will work closely with our community eating disorder teams, UHP, and other stakeholders to ensure seamless communication and collaboration.

As a Mental Health Liaison Practitioner, you will be responsible for:

  1. Supporting communication between UHP and community eating disorder teams within Plymouth, Devon, and Cornwall.
  2. Working autonomously with children and young people who are experiencing highly complex and persistent mental health difficulties.
  3. Establishing rapport and building effective working relationships with service users and all those involved in their psychological, emotional, and behavioural well-being.
  4. Communicating with parents/families/young people supporting the ward care plans and risk assessments.
  5. Supporting the ward staff team regarding any transfer of care to GAUs or SEDUs when required.
  6. Providing advice to young people and families around NG feeds.
  7. Offering ward staff supervision, CPD, and teaching around eating disorders and mealtime support.
  8. Attending and supporting monthly ED clinics with medical staff and other meetings where appropriate.
  9. Offering brief therapeutic on-ward input to young people and families in agreement with Plymouth, Devon, and Cornwall community teams.
  10. Supporting the adjustments required to care accordingly for young people with neurodiversity in liaison with their local team and ward staff.
  11. Contributing to the CAMHS Eating Disorder Pathway and being accountable to the Clinical Pathway Manager and lead clinicians for the CAMHS Eating Disorders Pathway.

Requirements

To be successful in this role, you will need:

  • Registered Nurse, Social Worker, or Occupational Therapist qualification.
  • Postgraduate diploma/degree-level study related to health/social care or equivalent worked experience relevant to the role.
  • At least 3 years post-qualifying experience in Secondary Mental Health, with 18 months in a community setting.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • High reflective capacity on own practice and commitment to life-long learning.
  • Flexible and resourceful in coping under pressure and with conflicting demands.
