About the Role

We are seeking a highly skilled and compassionate Adult Mental Health Practitioner to join our team at Livewell Southwest. As a key member of our community mental health team, you will play a vital role in providing high-quality care and support to individuals with mental health conditions.

Key Responsibilities
  • Assess and manage a caseload of patients with mental health conditions, providing individualized care and support.
  • Work collaboratively with multidisciplinary teams, including psychiatrists, mental health nurses, and other healthcare professionals.
  • Develop and implement personalized care plans, ensuring patients receive the best possible outcomes.
  • Liaise with other agencies, including social services, police, and healthcare providers, to ensure seamless care coordination.
  • Participate in daily team meetings and contribute to the development of care plans and strategies.
Requirements
  • Registered mental health or learning disabilities nurse or a final year student mental health nurse with previous experience in working in a mental health team.
  • Knowledge of the Mental Health Act, Personalised Care Framework, and NICE guidelines.
  • Ability to travel across Plymouth and work in a fast-paced community mental health setting.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
