About the Role

We are seeking a highly skilled and compassionate Substance Misuse Nurse to join our team at Change Grow Live. As a key member of our healthcare team, you will play a vital role in providing high-quality care and support to individuals struggling with substance misuse.

Key Responsibilities
  • Conduct comprehensive physical and mental health assessments for individuals accessing our services, identifying areas of need and developing effective care plans.
  • Lead the coordination and facilitation of our alcohol detoxification program, ensuring seamless transitions and optimal outcomes.
  • Contribute to our Naloxone Strategy by training staff and service users in the use of Naloxone, promoting harm reduction and overdose prevention.
  • Develop and maintain integrated care pathways for individuals requiring in-patient detoxification or onward referral for chronic or acute alcohol-related health needs.
  • Build strong therapeutic alliances with individuals struggling with substance misuse, fostering trust and promoting effective partnerships.
  • Ensure that procedures are regularly reviewed and updated to reflect best practice and evidence-based guidelines, aligning with UK clinical management standards and CGL protocols.
  • Provide harm reduction advice and support to injecting service users, ensuring safe equipment disposal and promoting healthy behaviors.
  • Support care coordinators with case management of individuals drinking at harmful and dependent levels, ensuring comprehensive care and support.
  • Record and input client data and information accurately, ensuring compliance with contractual, administrative, and financial requirements.
About You

To thrive in this role, we are looking for a highly skilled and dedicated Substance Misuse Nurse with:

  • A first-level registered nursing qualification and current PIN.
  • Recent relevant post-qualification experience in substance misuse treatment (desirable but not essential).
  • Sound knowledge of harm reduction and health promotion interventions.
  • Ability to assess and recovery plan service users, formulating written reports as necessary.
  • Clinical auditing skills to inform service evaluation and identify areas for development.
  • A professional, empathetic, and non-judgmental attitude towards service users and stakeholders.
  • An understanding of the importance of professional integrity in relationships with service users, peers, and other professionals.
