We are seeking a highly skilled and compassionate Substance Misuse Nurse to join our team at Change Grow Live. As a key member of our service, you will play a vital role in providing high-quality care and support to individuals struggling with substance misuse.
Key Responsibilities- Conduct comprehensive physical and mental health assessments for service users, identifying areas of need and developing effective care plans.
- Lead the coordination and facilitation of our alcohol detoxification program, ensuring seamless transitions and optimal outcomes.
- Contribute to our Naloxone Strategy by training staff and service users in its safe and effective use, promoting harm reduction and overdose prevention.
- Develop and maintain integrated care pathways for service users requiring in-patient detoxification or onward referral for chronic or acute alcohol-related health needs.
- Build strong therapeutic alliances with service users, fostering trust and empowering them to achieve their recovery goals.
- Ensure that our service operates in accordance with UK guidelines, NICE guidance, and CGL clinical protocols and procedures, promoting best practice and evidence-based care.
- Provide harm reduction advice and support to injecting service users, ensuring they have access to safe and effective equipment and resources.
- Support care coordinators with case management of individuals drinking at harmful and dependent levels, promoting holistic care and support.
- Record and input client data and information accurately, ensuring our service operates within contractual, administrative, and financial requirements.
To thrive in this role, we are looking for a highly skilled and dedicated Substance Misuse Nurse with:
- A current PIN and recent relevant post-qualification experience as a registered nurse.
- Relevant recent experience working within drug and alcohol treatment, with a sound understanding of harm reduction and health promotion interventions.
- The ability to assess and recovery plan service users, formulate written reports, and employ clinical auditing skills to inform service evaluation and development.
- A professional, empathetic, and non-judgmental attitude towards service users and all stakeholders, with a strong understanding of professional integrity in relationships.
- 25 days holiday (rising by 1 day for each year's service, pro rata) and paid 'Wellness' hour each week.
