Job Summary

We are seeking a highly skilled Advanced Respiratory Practitioner to join our Liverpool Community Respiratory Team. As a key member of our team, you will provide an integrated respiratory service across both primary and secondary care, working closely with patients, families, and healthcare professionals to deliver high-quality care.

Main Responsibilities
  • Assess patient conditions and develop comprehensive care plans to meet their needs, drawing on specialist services as required.
  • Undertake chronic disease management reviews of housebound patients, providing support and guidance to ensure optimal care.
  • Develop and implement individualized care plans, working collaboratively with patients, carers, and healthcare professionals to achieve the best possible outcomes.
  • Direct and support the management of complex and highly complex continuing care packages, ensuring seamless care coordination and delivery.

Person Specification
  • Master's degree or equivalent in a relevant discipline.
  • Registration with a relevant professional body.
  • Post-registration qualification or demonstrable equivalent experience.
  • Evidence of continuing professional development (CPD) and short courses.
Desirable Qualifications and Experience
  • Appropriate prescriber.
  • First-line management experience.
