About the Role

We are seeking an experienced Practitioner to join our team in providing efficient triage of referrals to ensure timely and equitable handling of all referrals.

Main Responsibilities
  • Clinical triage of referrals to determine eligibility for DPT LD services.
  • Liaising with stakeholders to assess patient level of Learning Disability and obtain consent for information sharing and treatment.
  • Participating in safeguarding to level 3 and coordinating care for individuals on the caseload.
  • Contributing to professional clinical governance arrangements and implementing audit and research findings.
  • Providing information, education, and advice to carers, families, and providers.
Requirements
  • Diploma or Degree in Community Practice, Behavioural Approaches, or Family and Systemic Therapy.
  • Evidence of ongoing training and development.
  • IT skills and ability to work in a hybrid environment.
