Job Title: Adults Mental Health Senior Practitioner

We are seeking a highly motivated and experienced Senior Mental Health Practitioner to join our team in the East & Mid Devon Home Treatment Team.

About the Role

As a Senior Mental Health Practitioner, you will be responsible for providing high-quality care to individuals with complex mental health needs. You will work collaboratively with a multi-disciplinary team to create care plans and strategies that deliver recovery-focused care.

Key Responsibilities
  • Develop and implement individualized care plans that promote recovery and well-being.
  • Provide expert clinical advice and guidance to colleagues and other healthcare professionals.
  • Conduct comprehensive assessments and risk assessments to inform care planning.
  • Work collaboratively with clients, families, and carers to ensure person-centered care.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements
  • Postgraduate qualification in mental health or a related field.
  • Substantial experience working in mental health services, preferably in a community setting.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Knowledge of recovery principles and mental health legislation.
