About the Role

We are seeking a highly motivated and experienced Senior Mental Health Practitioner to join our East & Mid Devon Home Treatment Team. As a key member of our multi-disciplinary team, you will be responsible for providing high-quality care to individuals with complex mental health needs.

Key Responsibilities
  • Develop and implement individualized care plans that promote recovery-focused care.
  • Work collaboratively with clients, their families, and carers to ensure effective communication and support.
  • Manage risk and make informed decisions to ensure the safety and well-being of clients.
  • Contribute to the development and implementation of team policies and procedures.
Requirements
  • Professional mental health qualification (RMN, CQSW / ASW, BSc/Diploma in OT, SROT, Chartered Clinical Psychologist, ENB 811/812 or CPN Degree).
  • Substantial experience working in mental health care, including inpatient and community settings.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
