About the Role

We are seeking a highly motivated and experienced Senior Mental Health Practitioner to join our East & Mid Devon Home Treatment Team. As a key member of our team, you will play a vital role in providing high-quality care to individuals with complex mental health needs.

Key Responsibilities
  • Develop and implement individualized care plans that promote recovery-focused care for people with complex mental health needs.
  • Work collaboratively with a multi-disciplinary team to create and implement care strategies that deliver effective outcomes.
  • Communicate effectively with clients, their families, and carers to ensure seamless care coordination.
  • Manage risk and develop creative solutions to maximize the effectiveness of care plans.
Requirements
  • Professional mental health qualification (RMN, CQSW / ASW, BSc/Diploma in OT, SROT, Chartered Clinical Psychologist, ENB 811/812 or CPN Degree).
  • Extensive post-registration experience working in mental health care, with a minimum of two years in inpatient and community settings.
  • Substantial experience of managing a busy caseload of clients with a range of mental health conditions.
  • Ability to clinically supervise junior and unqualified staff and mentor students.
