About the Role

We are seeking a highly motivated and enthusiastic Adult Mental Health Practitioner to join our East & Mid Devon Home Treatment Team. As a key member of our multi-disciplinary team, you will play a vital role in providing high-quality care to individuals with complex mental health needs.

Main Responsibilities
  • Develop and implement individualized recovery plans in collaboration with clients, their families, and carers.
  • Work closely with colleagues to create a supportive and inclusive environment that promotes recovery and well-being.
  • Communicate effectively with clients, their families, and carers, as well as other healthcare professionals.
  • Contribute to the development and implementation of care plans that prioritize recovery and well-being.
Requirements
  • A professional mental health qualification, such as RMN, Social Work Degree, or equivalent.
  • Experience working in a community mental health setting, preferably in a statutory or voluntary sector.
  • Ability to manage risk and plan creatively to maximize the effectiveness of care plans.
  • Awareness of community structures and social care management responsibilities.
