Paramedic Practitioner for Home Visiting Service

The Primary and Urgent Care Alliance is seeking an experienced Paramedic Practitioner to join our dynamic healthcare team in the Hastings and St Leonards area. As a key member of our team, you will play a crucial role in delivering high-quality patient-centred care through our Home Visiting Service.
Main Responsibilities- Assist in a comprehensive home visiting service, undertaking the visiting of patients who are unable to access their GP surgery.
- Assess, diagnose, treat, refer or direct patients to other services within the comfort of their own homes.
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
- Demonstrate safe, clinical decision making, acting within your professional boundaries and qualifications.
- Promote self-care and empower patients to make informed choices about their health and treatment.
- Initiate and interpret tests and investigations as appropriate.
This is an exciting opportunity to work as an autonomous practitioner in patient homes, working in collaboration with nine GP practices. You will have the full support of a remote clinical team and care coordinator to discuss cases and learning, asking for advice and learning is greatly encouraged.
Person Specification- Be able to commit to the rota, or set shifts per week if working with other Paramedics.
- Be a fully qualified Paramedic with a valid HCPC registration.
- Have at least two years post Newly Qualified Paramedic experience.
- Have a full driving license and own vehicle for home visits.
- Have experience or an interest supported by CPD in primary care.
