Paramedic Practitioner for Home Visiting Service

The Primary and Urgent Care Alliance is seeking a skilled Paramedic Practitioner to join our team in providing high-quality care through a Home Visiting Service in the Hastings and St Leonards area.
Main Responsibilities- Assist in a comprehensive home visiting service, undertaking the visiting of patients that are unable to access their GP surgery.
- Assess, diagnose, treat, refer or direct patients to other services within the comfort of their own homes.
- Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly.
- Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan.
- Demonstrate safe, clinical decision making, acting within your professional boundaries and qualifications.
- Promote self-care and empower patients to make informed choices about their health and treatment.
- Initiate and interpret tests and investigations as appropriate.
This is an exciting opportunity to work as an autonomous practitioner in patient homes, working in collaboration with nine GP practices. You will assess patients and commence treatment and/or refer them onward as appropriate. This service will help to reduce unplanned hospital admissions and actively monitor high-risk patients and initiate early interventions.
Requirements- Be a fully qualified Paramedic with a valid HCPC registration.
- Have at least two years of post-qualification experience.
- Possess a full driving license and own vehicle for home visits.
- Have experience or an interest in primary care, supported by CPD.
- Have excellent communication and interpersonal skills.
- Have strong clinical assessment and diagnostic skills.
- Be able to work independently and collaboratively within a multidisciplinary team.
- Be aware of data protection (GDPR) and confidentiality issues.
- Produce accurate and complete records of patient consultation, consistent with legislation, policies and procedures.
