Clinical Care Specialist for Home Visiting Service

About the Role
The Primary and Urgent Care Alliance is seeking a skilled Paramedic Practitioner to join our team and support our Home Visiting Service in the Hastings and St Leonards area.
Key Responsibilities
- Assess and diagnose patients in their own homes, providing high-quality care and treatment.
- Work collaboratively with a multidisciplinary team to deliver patient-centered care.
- Develop and implement individualized care plans to meet the needs of patients.
- Initiate and interpret tests and investigations as necessary.
- Provide education and support to patients to promote self-care and empower them to make informed decisions about their health.
Requirements
- At least two years of post-qualification experience as a Paramedic.
- Excellent communication and interpersonal skills.
- Strong clinical assessment and diagnostic skills.
- Ability to work independently and collaboratively within a multidisciplinary team.
- Full driving license and own vehicle for home visits.
