We are currently looking for three dedicated Outreach Support Specialists to enhance our teams in various locations. The successful candidates will provide exceptional services to groups that typically face barriers in accessing our support, utilizing the Making Every Contact Count (MECC) methodology.

This role will encompass proactive outreach and engagement efforts to facilitate access to treatment services for both current and prospective service users.

Locations: Various areas within the community.

Working Hours: Full-time, 37.5 hours per week

Salary: £27,449 - £31,529 annually, based on experience

Employment Type: Full-time

Key Responsibilities

Role Responsibilities:

As a vital member of our outreach team, your responsibilities will include:
  • Delivering outreach services focused on substance misuse to key locations and target groups, promoting harm reduction and facilitating swift access to treatment for those seeking assistance.
  • Collaborating closely with Accident & Emergency departments and other healthcare teams to create effective discharge pathways for frequent hospital attendees.
  • Establishing connections with specialized acute services and providing in-reach support to hospital wards.
  • Emphasizing early intervention and assertive engagement for individuals identified as needing additional support.
  • Working in partnership with various agencies, including GPs, social workers, and community mental health teams, while actively participating in community safety and vulnerability forums.
  • Ensuring case files reflect high-quality interventions, MECC principles, SMART goals, and clear outcome data.
  • Offering drop-in clinics at community venues.
  • Conducting comprehensive risk assessments and developing thorough health and safety management plans.

Candidate Profile:

To excel in this position, we seek individuals who possess:

  • Experience working within multidisciplinary teams and outreach settings.
  • The ability to work independently and maintain composure under pressure.
  • Proven expertise in client assessment, case management, and care coordination.
  • Experience in conducting dynamic risk assessments and formulating robust health and safety plans.
  • Emotional resilience and the capacity to apply diplomatic and practical approaches to managing priorities and challenging situations.
