Main Responsibilities

  • Communicate effectively with service-users, carers, and multi-disciplinary teams to ensure seamless care coordination.
  • Regularly report and document service-users' health status and care aims, ensuring accurate and timely information sharing.
  • Establish and maintain professional relationships with service-users, carers, and colleagues, adhering to trust policies and procedures.
  • Identify and mitigate risks of danger, harm, and abuse, reporting concerns to senior colleagues.
  • Work safely and effectively in community settings, minimizing risks to self and others.

Person Specification

We are looking for candidates with:

  • NVQ 3 in Mental Health and Social Care, with evidence of numeracy and literacy skills
  • Experience working with people with mental health needs in a community team setting
