Support Worker

We are seeking a highly motivated and compassionate Support Worker to join our team at Kent and Medway NHS and Social Care Partnership Trust. As a Support Worker, you will play a vital role in providing high-quality care and support to our service users, promoting their mental well-being and dignity.
Key Responsibilities- Assess and plan care alongside service users, taking into account their individual needs and goals.
- Liaise with Clinical Pathway Leads to deliver interventions and promote health and best practice.
- Contribute to assessing clients referred to Mental Health Together or Mental Health Together Plus.
- Work with individuals to facilitate access and engagement in leisure and community-based activities.
- Deliver a high standard of support to service users and their carers, promoting equality, dignity, and mental well-being.
- Previous experience in healthcare, preferably in mental health.
- NVQ Level 2 in Care or Community Mental Health Care, with a commitment to undertake a Level 3 qualification within an agreed time limit.
